Geriatric assessment
The A.G.E. Matters Geriatric assessment includes a comprehensive medical evaluation, an integrated, state-of-the-art cognitive assessment and a detailed functional assessment.
Personel include a Geriatric Medicine doctor, a Ph.D. Psychologist, and a certified Occupational Therapist, and an experienced Social Worker, specifically skilled in working with seniors.
Integrated Assessment:
Comprehensive medical evaluation by Dr. Tal is the bedrock of our clinic. We identify and optimize all aspects of general health, with a focus on cardiovascular problems.
Cognitive assessment begins with Dr. Tal, continues in-depth with the Psychologist, and is augmented further by the Occupational Therapist, who completes the assessment in a home visit, to assess the client's function in their home environment.
In addition to Cognitive assessment, Dr. Tal screens for issues of anxiety and depression. These are sometimes the primary problem, requiring specific treatment and restoration of normal cognition. Alternatively, anxiety and depression may be secondary problems, exacerbating the memory disorder. Again, careful assessment and specific therapy and treatment provides the best outcome.
Feedback session:
Here we integrate the assessments of the multidisciplinary team.
Dr. Tal presents and summarizes for the client and his/her support system. Questions are encouraged at this time, as well as an open discussion of all areas of concern. Treatment is recommended and initiated, including medications, life-style modifications, and cognitive training.
Follow up is arranged as appropriate.
Referrals:
Referral may be from your family doctor or from a specialist.
Referrals may also be direct, from an individual, a family member or a significant person in their life.
